I, [Parent/Legal Guardian], hereby grant permission for my minor child, to be photographed or videotaped during their participation in the production of Peter Pan at Home Made Theater, Inc.

I understand and agree to the following terms and conditions:

Consent: I grant permission for Home Made Theater, Inc. to capture, reproduce, and use photographs and/or video footage of my child for the following purposes: promotional materials, website content, social media, newsletters, press releases, educational materials, and any other lawful purposes related to the theater/production.

Release: I release and discharge Home Made Theater, Inc. and its representatives, employees, agents, and affiliates from any claims, demands, or liabilities arising out of or related to the use of the photographs or video footage, including any claims for defamation, invasion of privacy, or infringement of moral rights.

Ownership: I understand and agree that all photographs and video footage taken by Home Made Theater, Inc. shall be the property of Home Made Theater, Inc., and they may use, edit, reproduce, and distribute the images at their discretion.

Confidentiality: I acknowledge that Home Made Theater, Inc. will make reasonable efforts to protect the privacy of my child by not associating their full name or personal contact information directly with any photographs or video footage used.

Compensation: I understand that my child will not receive any compensation for the use of their photographs or video footage.

Revocation: I have the right to revoke this release at any time by providing written notice to Home Made Theater, Inc. However, I understand that any photographs or video footage taken prior to the revocation may still be used as outlined in this release.

Governing Law: This release shall be governed by and construed in accordance with the laws of the State of New York.

I have read this Photo Release Form carefully and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
